Please find below instructions for certificate retrieval:

1. For products with lot numbers beginning with 06, 07, or 08 (e.g., 08MLK001), please retrieve your Certificate of Sterility at the Cardinal Health website at http://sterility.cardinal.com or contact DuPont Customer Service at 1-800-931-3456.
2. For products with lot numbers beginning with "M" (e.g., MDCS00123 or M00S12345), please contact DuPont at personalprotection@usa.dupont.com or contact DuPont Customer Service at 1-800-931-3456.
3. For products with lot numbers beginning with "R" (e.g., RDCS00123 or R00S12345), please input your information below.

After inputting your user information below, you will see a search screen. From there, you can search to find the specific sterility certificate you need. Please note that, to print or view the sterility certificates on this site, you will need Adobe® Reader® or an equivalent pdf file viewer. Click here to download it.
